We have a single email address for all queries (but not referrals or patient-identifiable information). Do not send patient-identifiable information to this email address:

The form is below. Sending patient-identifiable information via this form is likely to represent a breach of patient confidentiality and data protection. Remember that date of birth or CHI is patient-identifiable information. The CHI not only contains the date of birth but also the sex of the patient.
Instead, send a general enquiry and we will reply to you from an NHS email address that permits confidential information to be transferred if you are also using an NHS email address.
