Rating scales to complete online

Several rating scales can be completed online. This means that you don't have to fill in a paper copy and post it back to us. If we have suggested that you fill it in online, we will have given you a unique ID that allows us to connect the completed form with you. This ID is unique to you, and can't be traced back. It is different to your CHI number. It should be entered into the form when you complete it.

The EQ-5D is a self-report measure of health status. It also includes a 'visual analogue' scale, so that the individual can rate their health from a 0 (worst) to 100 (best) state. 

The WHODAS 2.0 is a scale developed by the World Health Organisation to measure difficulties caused by any condition. It measures the effects of a disorder on someone's ability to do everyday things. It consists of 36 questions.
