The ATHF consists of scoring instructions and ratings for most antidepressants/ augmentation/ and ECT trials. It is being used increasingly to determine the adequacy of antidepressant trials.

These forms can be used for completing medication reviews for patients with depression, OCD, and bipolar disorder.
In most cases, we would advise using the MS Word version since this will automatically expand to accommodate additional information. Instructions for completion are included in each form. The form for depression assumes that you will be using the ATHF to score treatment trials (see above).
This proforma permits a systematic recording of all previous treatments for major depression, along with details on illness history and admission record.
